My mom took out an extension of her credit card with my name does this affect my credit?

I recently received a call from collections, about an extension of a credit card that I did not take out. My mom took it out with out my knowledge or authorization she apparently has maxed it out and also let it go to collections. I don't know what to do because now my name is on this and I am very worried about my credit because of what she did. I don't know how to go about this and how to get my name out if this situation anyone know if this extension affects my credit she apparently is the primary account holder and she took this out to get more points.
